The translation follows:

It happened in the village of Lipniki, on October 6th/19th, 1900, at 9 a.m.
Appeared Jan Lis, 60 years old and Józef Kisiel, 48 years old, both farmers living in Lipniki and they declared that in Lipniki, yesterday, at 10 p.m., died Józef Zduńczyk, 56 years old, a farmer, living in Lipniki, a son of the late Franciszek and Ewa née Marchewka, the Zduńczyk spouses.
